PERKINS, Constance Mary "Connie", 76, passed away Tuesday, June 30, 2009 in Tulsa, OK. She was born May 3, 1933 in Kansas City, MO to Dr. Stan J. and Cecilia Sulkowski who preceded her in death. She is survived by daughter; Lori Stevens and husband Ron, and grandchildren; Elizabeth and Matthew Stevens, all of Tulsa, OK. A Rosary will be said Thursday, July 2, 2009 at Fitzgerald Southwood Colonial Chapel and a Mass of Christian Burial will be held 10:00am Friday,, July 3, 2009 at St. Bernard of Clairvaux Church. Fitzgerald Southwood Colonial Chapel.
